Dear Residents, Relatives and Carers

As a facility we understand that Person Centred Care is essential in ensuring that individuals feel safe, respected, acknowledged and treated with dignity. In line with the change in Aged Care Standards the Aged Care Quality and Safety Commission (ACQSC) is focused on ensuring trauma-informed care is incorporated in everyone’s Care and Services Plan, particularly for individuals who have experienced trauma.

This approach acknowledges that a significant number of older people may have experienced trauma, which can affect their well-being and how they interact with care services. By adopting trauma-informed practices, the ACQSC aims to prevent re-traumatisation, promote healing, and improve the overall quality of life for older people in care.

We have generated the attached booklet for residents, families and carers to complete upon admission to the facility, however for the residents already residing at the facility we request that this be completed by the most appropriate person and sent back to our Care Managers Hannah Devenport – email or Kristine Latoja – so that this can be added to each individuals Care and Services plan.

We understand that completing this form, which relates to such an important aspect of someone’s life, may be challenging. If you require any assistance, please feel free to reach out to the staff member you feel most comfortable with within the facility.
